Poteau, OK – Eastern Oklahoma Medical Center (EOMC) has been awarded a 5 year [grant] through the Oklahoma Rural Health Transformation Program (RHTP) to strengthen healthcare access and improve health outcomes in Leflore County Region.

“This funding allows us to reach more patients” said Tiffany Griffis, CEO. “We are proud to partner with the Oklahoma Rural Health Transformation Program to improve the health of our rural communities.”

The Oklahoma RHTP is part of a historic $50 billion federal investment by the Centers for Medicare & Medicaid Services (CMS) authorized by the One Big Beautiful Bill Act. Oklahoma secured $223,476,948.62 million for the first budget period of a five-year grant that focuses on helping people get the care they need where they live, leading to a healthier, thriving state.

As part of the RHTP, all awarded projects will benefit a rural community and be designed with long-term sustainability strategies in mind.

Through this investment, EOMC aims to use this opportunity to improve the health and well-being of the communities we serve by connecting patients with the resources, education, and support they need to achieve better health outcomes. Our goal is to address barriers to care, promote preventive health, and help patients better manage chronic conditions such as hypertension and diabetes through personalized guidance, ongoing support, and coordination with healthcare providers.

By meeting patients where they are and helping them navigate the healthcare system, our Community Health Worker Program aims to improve access to care, reduce preventable hospital visits, encourage healthier lifestyles, and support patients in taking an active role in their health. Through collaboration with local healthcare providers, community organizations, and other support services, EOMC is working to build a healthier, stronger community where every patient can live a healthier and more independent life.
