OSU National Center for Wellness & Recovery OSU Medicine and OSU Cooperative Extension Service invite you to participate in a COVID-19 Impact Survey 

What are we asking you to do? 

You are invited to participate in a web-based online survey to learn more about the effects of COVID-19 also known as the novel coronavirus pandemic on families and communities. This research project is being conducted by Oklahoma State University (OSU) Cooperative Extension Services (OCES) in partnership with the OSU National Center for Wellness and Recovery (NCWR).

We are asking you to take a one-time survey to share your experience. The survey will ask about how COVID-19 has impacted day to day life, attitudes and behaviors such as, managing stress and substance use, etc. The survey should take approximately 15-20 minutes to complete. The results of this survey will be used by your local Extension Office to better direct resources to your community. We appreciate your efforts to improve your community.

Please remember that your participation in this survey is voluntary. You may refuse to take part in the research or exit the survey at any time without penalty. You are free to skip any particular question(s) you do not wish to answer for any reason.

Possible Benefits of Participation 

You will receive no direct benefits from participating in this research study. However, your responses may help us better direct resources to your community, and we hope that anyone who participates feels good about helping others.

Foreseeable Risks of Participation 

There are no foreseeable risks involved in participating in this study other than those encountered in day-to-day life. However, we have taken several steps to ensure your confidentiality.

  • Your survey answers will be sent to a link at where data will be stored in a password protected electronic format.
  • Qualtrics does not collect identifying information such as your name, email address, or IP address. Therefore, your responses will remain anonymous.
  • No one will be able to identify you or your answers, and the research team will not know whether or not you participated in the study.

All records related to this study will be destroyed after the study is completed (approximately 08/01/2021).
